The following information is provided as a pre-qualification sheet and to provide assistance in downloading the appropriate forms for enrollment should you decide to join the Manatee Division.

Naval Sea Cadet Qualifications for Enrollment

• Must be a U. S. Citizen
• 13 years old and not yet have reached their 18th birthday (Sea Cadets)
• 11 to 13 years old (Navy League Cadets. Waivers may be available to highly motivated and mature candidates 10 years old.)
  
• Must complete a medical examination by a school or personal physician
• Must be enrolled in school/home schooled and have a satisfactory scholastic standing
• Must have a good moral character and be free of felony conviction
• Must be single and remain single to be enrolled

Enrollment Fees

• Annual NHQ Enrollment Fee $53.00 (includes liability insurance)
• Uniform Rental Fee $100.00
• Unit Enrollment Fee $22.00
• Please submit the total fee of $175 with the applications below. Payment options may be available.
  

Enrollment Process:

If you meet the qualifications, here is what you need to do next:

1. Fill out the forms below and bring them to the next drill weekend. Call or email for drill date details.
2. Parents or Guardians should accompany their child to the first drill to talk our admin officer.
3. Bring a copy of their Health Insurance Card.
   

Note: All forms can be filled out online and printed. You can also print them out and fill them in manually.  You will need Adobe Acrobat Reader to read and fill in the forms.

NSCADM 001-Cadet Application and Agreement (Please read carefully and sign all appropriate spaces)
NSCADM 004-Parental Agreement
NSCADM 020-Report of Medical History. Please list all allergies and medications. This must be signed by a licensed physician. If using the latest school physical, a copy of it is allowed, but this form must be signed by a physician.
